EFT Cancellation procedures

1. Cancellations can only be processed after the 2-month minimum automatic transfer. Early termination fees may apply.
2. To be effective on the 1st of the month, cancellation requests must be received in writing by the 15th of the month before.
3. Cancellation requests may be made in person at Mountain Sky Jazzercise, via US Mail or via E-mail. You will receive a confirmation of your cancellation via US mail from Mountain Sky Jazzercise after the 15th of the month. If you do not receive a confirmation, please contact Mountain Sky Jazzercise to ensure your request was received. (Telephone requests can not be processed.)
4. Requests made after the 15th of the month will not be effective until the first of the subsequent month. (a cancel request received on January 16th  would be effective March 1st)
5. If you forget or delay in giving written notice, monthly transfers will continue and are not refundable.  If you are simply not attending class we cannot assume you are never coming back.  Absence from class is not considered notification. It is your responsibility to notify us so we may act on your behalf.
